Deathsmiles is a horizontal shoot'em up from Cave on Cave CV1000, this title is resolutely wacky by involving witches, giant cows and other original creatures.

Unlike the life system on classic shoot-ups, the player has a certain amount of life points (3 by default). 1 point is lost when the player is hit by an enemy ball, 1/2 point for another type of enemy attack or for a collision with an enemy. The game ends when the player loses all of his life points. These health points can be restored by items, and the maximum number of life points can be increased when the player reaches certain scores (by default, 20 and 45 million points).

Mega Black Label is a limited (200 copies total) arcade edition of the game released in Japan. It added Sakura as playable character, as well as the Crystal Shrine stage, and Level 999 difficulty level. There are other scoring changes.
